Video and Survey: Letting NHS staff know you need support – Reasonable adjustments, flagging and summary care records

How health services share information about support for people when they need reasonable adjustments when visiting health services. This video talks about how using a flag or a note on your computer health record and extra information in your Summary Care Record can help. This will be piloted for people with a learning disability before being used for other people. Please fill in this survey about what information you would like your GP to share with other health services about your support needs (or reasonable adjustments) – if you have a learning disability, or if you care for someone with a learning disability https://tinyurl.com/NHSFlagSurvey Here is a consent form and information that you can fill in to give to your GP and here is one in Easy Read. https://digital.nhs.uk/summary-care-r… https://www.mencap.org.uk/sites/defau… This video has been produced by the Learning Disability team at NHS England.
