Transforming Care Job: Governance Case Manager for Sussex

An exciting opportunity has arisen for a Governance Case Manager across Sussex that focuses upon improving the care, treatment and support of our clients that are part of our  Transforming Care Programme. A major focus will be on ensuring safe and timely discharges of this client group.

If you are keen to further your professional experience and demonstrate your ability to work across organisational boundaries and influence care pathways this could be the role for you.

The role is [AfC] Band 8a or equivalent, and will be for one year.

You will need to

  • Have current Registered Nurse/Social Worker or AHP status or other relevant qualifications and experience in a senior position managing complex case work
  • have knowledge of the legal and therapeutic services relating to learning disabilities and autism
  • take decisions on difficult or contentious issues where there maybe a number of courses of action
  • Case management skills
  • Completer/Finisher
  • To be a critical friend
