TLAP: Making it Real

Making it Real is a framework for how to do personalised care and support. It’s for people working in health, adult social care, housing, and  for people who access services.

It is an easy to use, jargon-free set of personalised principles that focus on what matters to people. Making it Real is built around six themes. These describe what good looks like from an individual’s perspective and what organisations should be doing to live up to those expectations. It supports co-production between people, commissioners and providers.

Making it Real is a voluntary commitment with access to free materials and support. If you sign up to get involved, you decide how to make best use of it.

Making it Real has been co-produced by Think Local Act Personal and the Coalition for Collaborative Care, with input from partners, organisations and individuals, including the National Co-production Advisory Group (NCAG).

VCSE: Inclusion Health Audit Tool

This online tool will help your organisation to audit it’s engagement with Inclusion Health groups. These are the groups identified as experiencing the worst health inequalities in the UK.

The tool consists of five sections and takes around 15 minutes to complete. Once you have completed the audit tool, you will be provided with a unique and tailored guide which will help your organisation to embed action on tackling health inequalities into its everyday activities.

Hft submits evidence to HoL Economic Affairs Committee inquiry into social care funding

Hft, a national charity that supports adults with learning disabilities, has submitted evidence to the House of Lords Economic Affairs Committee inquiry into the funding of adult social care. In its submission, the charity warned that financial pressures in the learning disability sector are often overlooked in a conversation dominated by elderly care.
